Last week, MPP for Elgin-Middlesex-London, Rob Flack, visited with the VON Middlesex-Elgin Team to talk about their $146,300 Resilient Communities Fund grant from the Ontario Trillium Foundation. The grant will help support VON Connect, to deliver care to more clients and their families in more communities. The grant will help them move forward with a digital health strategy that will help meet the growing demand for home and community care more effectively, with more family and client engagement. This transition to a digital health care portal will be completed in 2024 and will address efficiencies to help meet the growing demand for home and community care in London-Middlesex-Elgin.
VON Connect allows providers to do what they do best – providing excellent care – rather than filling out paperwork, and because VON also provides community support services, they are able to refer clients easily and seamlessly to other services and programs that will further promote their health and well-being.
