Older adults in Ailsa Craig and nearby rural communities now have better access to health care and support services thanks to a new provincial program at Craigwiel Gardens.
Craigwiel Gardens has been chosen to take part in Ontario’s Community Access to Long-Term Care (CALTC) pilot program. The funding will help expand the organization’s Connections program, making it easier for seniors to stay healthy and live safely in their own homes for longer.
Craigwiel Gardens President and CEO Elaine Shantz says people living in rural communities can face extra challenges when trying to access specialized care. She says the new program will provide easier access to services such as clinical care, community paramedicine, memory care, and other supports that help seniors remain independent.
The Connections program is available to people who are on, or may qualify for, the long-term care waitlist. It also helps people who need extra support to avoid a hospital stay or delay moving into long-term care.
The program also includes a MINT Memory Clinic, which offers care and support for people living with memory loss or dementia so they can continue living well at home.
People can now refer themselves to the Connections program or be referred by a family member, caregiver, doctor, health-care provider, or community organization. Referrals can be made by contacting Craigwiel Gardens directly or by completing a form on the Craigwiel Gardens website.
Appointments at the MINT Memory Clinic require a referral from a physician, specialist, or nurse practitioner.
